Apparently it is supposed to be 26 episodes, which is absolutely insane for how slow the series has been progressing, unless there is a) very little plot to come, b) more seasons planned or c) the plot really speeds up.

After reading your past thoughts on Letter Bee, I am debating on whether to keep this series or drop it.

The patient is a former child actor whose problem is that he is a compulsive smiler due to the signature character he once played. Later in the episode he discovers that the only time he doesn't smile is when he is in pain, so to prevent himself from smiling too much during an audition, he clamps clothespins to his nipples.

And now you know the story.

I will admit, though, that the image of the guy ripping off his shirt, with the clothespins he had on his nipples flying off (don't ask), is hilarious.
