The vectoring isn't terrible, but the places that were overlooked or forgotten are glaringly obvious. The background isn't bad either, but it looks very rough in places.

I think, for the most part you did vector it! ^_^ But I see see around the outlines and places in the eyes that you didn't, lol. For a full vector, you must also do the outlines and everything. :) But this is really close! And it's amazing that you used that original scan...that was horrible quality! So you did make it better. ^^ You can just see some pixelation still around her face the the places you didn't vector. [because with a vector, you would take away the original scan's layer, and you would just have left all that you redid]

That's a lot of layers! XD Lemme tell you some tips about that. Did you use the shape pen tool or the path pen tool? Because there are two different options to choose. Usually the path one is best for outlines, and then shape for coloring. At the top of photoshop, when you have them selected (usually just the shape pen tool) you can select to have your layers combine each time you do a new shape. Or you could just press the + key (shift and = ) after you've done your first shape layer. Then you won't have so many!
