I have a concern with one of your questions. In the question, "What type of anime/manga do you watch/read?", you gave the instruction to "choose at least 2 and at most 2 answers". I'm not sure if this is supposed to be for amusement, but you may want to simplify this into "choose TWO answers ONLY" or something like that. There's no need to make it as complicated as it is.
