I believe Mushishi is an underrated anime with a lot of strange, mysterious happenings in the show. I was left wondering a lot about who Ginko WAS exactly, his background and the mystery of how he became the way he was.
One of my favorites came at the end of the Vampire Princess Miyu TV series with the shocking revelation that the scene we've been seeing in the end credit sequence of Miyu holding the severed head turns out to be the final scene of the series! The credits have been showing us how it would end all along! Sneaky!
While watching ergo proxy There were alot of mysterious things going on like When pino visited one of the domes through a dream or when the group went to the caves and such to search for the memories that Vincent aka the death proxy lost.

I've seen a lot of mysterious elements in the multiple shows and stuff I've watched over the years.
The most mysterious event I've seen in an anime recently was in Yu-Gi-Oh ARC-V. There have been multiple mysterious events that have thrown me for a loop since I've started watching it.
The most mysterious event so far I've seen so far in Yu-Gi-Oh ARC-V was when Yuri was pursuing Yuzu and Yugo appeared just in time and Yuri disappeared due to Yuzu's mysterious bracelet. This bracelet has an air of mystery about it, there are three other girls who have bracelets with unknown powers similar to Yuzu's. But that's the one that sticks out to me so far.

A mysterious event? When Shinichi Kudo was given a strange pill that caused him to shrink down into a child and became Conan Edogawa due to the mysterious Black Organization. It's been since 1994 when the Detective Conan manga started and we still don't know all the secrets of the Black Organization! It's also a complete mystery to how someone almost always dies when Conan is around.
Oh gosh it took me a bit to think about this, but I would have to say Baccano did it for me. I don't want to say too much, but the whole anime (just 12 episodes!!) was full of mystery. Throughout the whole thing I was pretty confused, it jumped from year to year, looking at the past and current. It wasn't until the end that everything made sense. But I guess the biggest mystery was who the Rail Tracer was and what was up with the immortals.
My favorite mysterious event in an anime was Lil Slugger in Paranoia Agent.
It's silly now but watching it when I was younger gave me the chills! Who was this kid with a baseball bat that kept attacking people that no one could catch?? It would keep me up and any foreshadowing of him in an episode would get my heart racing.
Hmmm...for me, it's gotta be when Keichi dies in Higurashi no Naku Koro ni-Wataganashi-hen ^~^; Because then we're left with the question of who killed Keichi ;A; Was it Shion? Mion? A third party? @_@
