Actually, when Cloud approaches Aerith, his mind was controlled by Sephiroth. Sephiroth tried to use Cloud kill Aerith. When Cloud raises his Buster Sword to slice Aerith, Tifa and the others stopped him by shouting at him. The moment Cloud regained his consciousness, Sephiroth has jumped towards Aerith with his Masamune aiming at Aerith heart. It was too late for Cloud to take any action to stop Sephiroth at that moment. In the end, Aerith died...
That's the reason why Cloud cannot forgive himself in Advent Children...
Not only because he cannot save Aerith, but also because he approached Aerith just to kill her in the first place... He thinks that if it wasn't Sephiroth, then he is the one who killed Aerith...
